It can feel a bit overwhelming to start the process for your son or daughter to submit their paperwork, but here are 12 things that you can get ready ahead of time to make the process of submitting paperwork less stressful!
What things do I need to have ready in order to submit my mission papers?
Identification
- Passport: If your missionary has one (and you can read here why I recommend they get one BEFORE filling out their paperwork, but don’t delay filling out paperworkuntil you have one…I just think they are great to get ahead of time). You will need the expiration date and passport number.
- Driver’s License: you will need to provide driver’s license ID number, country and expiration date.
Health Information
3. Health Insurance Card: If they are double insured have both!
4. Health History Surgery: This includes past surgeries and surgery dates
5. Health History: Current medication (both Over-the-counter and prescription). For medication you will need to know exact names and dosage so pull out those medicine bottles!
Scan These Items
6. Photo of your missionary with white background, dressed and groomed according to missionary standards. This will be part of what the apostle/committee looks at while they make their decision.
7. Front and back of Driver’s License
8. Passport (if your missionary has one)
9. Medical documents that clarify any conditions listed in Personal Health Information
Print these documents from the missionary portal
10. Completed Personal Health History to give physician: Have your missionary fill this out on their own! They will give it to their physician during their pre-mission physical. It will bring up any concerns that need to be addressed before their mission. SO IMPORTANT! You don’t want your missionary serving somewhere where they don’t have access to the things they need to be physically and/or mentally well.
11. Physician Evaluation Packet: This is the packet that your missionary will give to the physician at their physical. The doctor fills out the packet and submits it (instructions are included on how they get it to headquarters).
12. Dentist Evaluation Form: Similar to the physician packet, this is the document your missionary will give to dentist at their check-up.
